​Scheduling an Appointment:

With real time appointment scheduling, you can schedule an appointment at your convenience with our online option.  Can't find an appointment slot, call us and we will try to find a time that works for you!  Please complete patient forms prior to visit.
As we re-open, please call our office for appointments.  It is important to understand the changes that have been implemented within the clinic.

What to Bring?

​What to bring to your appointment:

The doctors at All Eyecare Optometry care about making sure you see your best, and that you preserve your eye health.  During your appointment, your eye doctor will write a glasses prescription for you, update your contact lenses, and check to make sure all of your eyes' structures are healthy. As a result, please expect your eye doctor to either have your eyes dilated or an Optomap photo taken of the retina during your comprehensive eye examination. 

What to bring to your appointment if you are a new patient:
- A copy of your driver's license / ID 
- A copy of your medical insurance card (e.g. Aetna, Anthem, Blue Cross, Medicare)
- Your old glasses or glasses prescription if available
- Empty contact lens packets or contact lens prescription if available 
- New patient forms (click here to access)
- List of your medications, including supplements, and allergies ​
What is Vision Insurance?

Understanding Vision Insurance

Let’s face it, insurance can be confusing. This is particularly true when an individual has both medical and vision coverage. Understanding your insurance PRIOR to any service can help you avoid confusion and frustration. We would like to avoid any misconceptions, and we hope that the following will help you better understand how your insurance coverage works.
Most vision plans to do not cover any medical testing, diagnosis, consultation or treatment. Vision insurance covers only  routine eye exams for purchasing glasses or fitting and purchasing contact lenses.

Many vision insurances do not send you a benefit card. If you have benefits through your employer your HR representative can tell you who what company provides your vision insurance. For self purchased plans you can call the customer service number on the back of your medical card and their representatives can look up if you have vision coverage attached with that plan.

​Regardless of your vision insurance, most plans do not cover 100% of expenses, and thus you should expect some out-of-pocket costs. There may be co-pays, deductibles or a percentage of costs that you will pay out-of-pocket as required by your insurance policy. As with most doctors, at All EyeCare the patient’s portion must be paid before materials (glasses or contacts lens) can be ordered. And all co-pays are due at the time services are rendered.

What is Medical Insurance?

Understanding Medical Insurance

Medical insurance DOES NOT cover vision   related issues such as routine exams,   glasses, and contact lenses. Many people with medical insurance have a separate rider policy to cover routine eye exams.  

​MEDICAL concerns (Glaucoma, Dry Eyes, Macular Degeneration, Red-Eyes, Floaters, Allergic Conjunctivitis) take priority and as such will be treated first or concurrently with a vision problem. Sometimes a medical condition has to be treated and corrected before vision can be accurately evaluated. Medical insurance companies usually separate the components of an eye exam, one being the comprehensive exam and the other being the refraction. (The refraction determines the prescription for eyeglasses and contacts.) You are responsible for the cost of the refraction if your insurance is medical only.
 
​If the presence of disease is detected that require additional testing, the doctor will provide you information regarding the condition and the testing required.  Depending on your insurance policy, deductibles and coverage services rendered may have some additional out-of-pocket costs that cannot be determined until your insurance company has been billed and provides an eligibility of benefits (EOB).  Regardless, you will be responsible for any deductibles our out-of-pocket costs. 

Although our staff members are very knowledgeable about insurance plans, remember that it is your responsibility to know the details of your individual health plan, including information about your deductibles and co-pays. Your insurance is a contract between you, your employer, and the insurance company, and not with the doctor. We encourage you to speak with your insurance company PRIOR to your appointment about your plan's specific details. Then, as always, feel free to ask us questions about how they will apply to your upcoming visit. We will do everything we can to help you better understand your policy, but the more knowledge you have about how it works ahead of time, the less frustrating it will be for you at the time of the exam.

Patient Forms:

Please complete the patient registration prior to your visit in order to be able to allow adequate time for the doctor to review your chart and needs.  Due to the recent COVID-19 outbreak, patients will not be able to complete forms in office to reduce exposure time.  As a result, your appointment may be rescheduled if the forms are not completed.  Thank you for your time and understanding.
